Welcome to your PADI Diver Training Course! Your diver training is designed to be fun and exciting. This course will however, require an investment of your time and due diligence, to ensure your independent study is learned and completed. This Learning Agreement is between you, (the student), Your Instructor, and Professional Assistant’s to your Instructor; regarding our mutual responsibilities in completing your PADI training program.
STUDENT RESPONSIBILITIES
1. Prior to your first in class session, completely read and sign the following:
2. Answer the Medical History questionnaire honestly by placing an “X” in the “YES” or “NO” in each question space on page #1 and Page #2 as applicable. If you answer YES to questions 3, 5 or 10 from page #1 OR, to any of the questions on page #2; sign and date it, and take all three pages of this form (Participant Questionnaire and the Physician’s Evaluation Form) to your physician for a medical evaluation. If a medical evaluation is required, you will require a medical clearance BEFORE ANY IN-WATER TRAINING.
3. COMPLETE YOUR INDEPENDENT STUDY PRIOR TO YOUR SCHEDULED CLASS START DATE. This means:
4. Your Instructor reserves the right to reschedule training sessions for any student that has not;
5. Provide a diving logbook before the first class for your instructor’s signature. This logbook is required material and should accompany you to all training events. If you lose this logbook, you will be required to purchase another in order to maintain the tracking of your training.
6. Possess an appropriate scuba quality mask, snorkel, fins, booties, gloves before the first in water training session.
7. Follow all course procedures and directions from your instructor and, ask questions about your training anytime
8. Accept financial responsibility for any lost, damaged, or missing rental scuba equipment assigned to you during the class and return all rental equipment to by the following day after the training session, unless otherwise directed by your instructor.
9. Be aware that YOUR INSTRUCTOR requires students complete the course within six (6) months of the start of the class to ensure knowledge retention. If you fail to complete the course within six months YOUR INSTRUCTOR reserves the right to require you to repeat the entire class at full price to achieve certification.
INSTRUCTOR RESPONSIBILITIES
1. Provide a schedule of training sessions and start the class as scheduled unless Open or Confined water conditions prohibit diving.
2. Provide a positive learning environment in which to master the course objectives.
3. Provide a telephone or contact number(s) for students to make direct contact with their instructor(s) for notifications in the event of being late or absent, or to ask general questions related to their training.
4. Answer the student’s question to the best of their ability and training.
5. Assist the student through all challenges.
If the student completes all course work assigned, arrives for training sessions promptly and follows directions for learning as assigned by Instructors, we accept responsibility for reasonable learning challenges that may occur. Course objectives must be met before the student is certified. Occasionally, a student enrolled in a group class experiences such significant learning challenges that they may be unable to achieve the performance requirement at the same pace as other class members. If remedial training required by a student, would prevent other students from completing the training within the allotted time, YOUR INSTRUCTOR reserves the right to provide private lessons (at private lesson rates) or re-enroll the students in subsequent group class to assist them in overcoming their learning difficulties.
Course Paperwork You will receive an E-Mail containing Course Paperwork. All course paperwork must be signed and turned in before any training begins. All minors must have a Parent or Guardian signature. No Exceptions. Note: you will be charged a reschedule fee if you cannot participate in the class due to not having your paperwork completed.
Rental Equipment This is an additional charge of $60.00 per day and includes the following equipment;
· Wetsuit
· Buoyancy Control Devise BCD
· 1st and 2ndstage Regulator with alternate air source, depth gauge, and submersible pressure gauge
· Scuba Cylinder and Air
· Weights
Personal Equipment You will be required to have the following personal items not included in the rental equipment suite. Note: these personal items must be approved for use by your instructor for use during your course. If not approved, you will be required to obtain these items before beginning your course, and will be charged a rescheduling fee.
· Mask
· Fins
· Snorkel
· Boots
· Gloves
These items may be purchased from GBS at a 10% discount while you are enrolled in a course with us.
Additional Training Costs In the event additional training is required for you to master requirements, additional training costs may incur. The following additional charges may apply:
· Exams and classroom sessions $60
· Confined water training session $150
· Open Water Training $150 per dive
· Entry fees apply and are site dependent
· Boat fees apply and are dependent
Arrival and check in for Training It is very important to arrive on time for your training to avoid rescheduling fees. Arrive early and have all of the required equipment, paperwork, and materials.
Late Arrivals Failing to arrive and check-in in person at the proper time, will be considered a no-show. It is at the instructor’s discretion whether to let a student continue their course after a late arrival. GBS and their instructors, reserves the right to fill all unclaimed spot within 15 minutes of the start of class or 30 minutes prior to boat dives with standby customers. If you lose your spot to a standby customer and wish to continue your training you will have to pay a reschedule fee and join the next available class where you left off.
Diver fitness No drugs or alcohol allowed during any activities or events. Improper, rude or violent behavior will not be tolerated and will terminate your activity. GBS and its instructors, reserve the right to terminate your participation due to safety concerns with use of alcohol and or drugs, or disruptive behavior. If removed from activities under these circumstances, there are no refunds or credits.
Acknowledgement I have completely read and fully understand the above terms and conditions. I also completely agree to the refund, cancellation, and rescheduling policies. By purchasing equipment, booking a course, or dive activities; you accept and agree to all the terms and conditions of this agreement.
Return Policy
All merchandise sales are final. GBS will not issue refunds for merchandise purchased. A store credit may be applied, provided the returned merchandise has not been damaged, used, and is still in its original packaging; minus a 10% restocking fee.
All sales for courses of instruction are final and must be completed within the specified time frame for each course. If the course is not completed within the specified time frame, the student will have to purchase the course again, in its entirety, at the current rate.
For all courses, academic costs (PADI Costs) are nonrefundable. GBS may issue a refund for the cost of unused classroom, and/ or in water activities only, so long as these activities were unable to be completed due to weather, illness, legal obligation, or cancellations by GBS; and the request for a refund with cause, is provided to GBS within 24 hours of the activity.
If the activity has to be rescheduled, a $95.00 rescheduling fee will be required for each occurrence; excluding circumstances due to weather, illness, legal obligation, or cancellations by GBS; and the request for a refund with cause, is provided to GBS within 24 hours of the activity.
All sales for dive activities are final. GBS may issue a refund for these activities, so long as these activities were unable to be completed due to weather, illness, legal obligation, or cancellations by GBS; and the request for a refund with cause is provided to GBS within 24 hours of the activity.
